The differential diagnosis of Sjögren’s syndrome includes other diseases which may cause dry eyes or mouth or parotid salivary gland enlargement

DIFFERENTIAL DIAGNOSIS OF SICCA SYMPTOMS

1. PAROTID GLAND ENLARGEMENT
• Bilateral Viral infections : mumps, influenza, Eptsein-Barr, coxsackie A, cytomegalovirus, HIV;
-Sarcoidosis;
-Amyloidosis;
-Sjögren’s syndrome;
-Metabolic : diabetes mellitus, hyperlipoproteinemias, chronic pancreatitis, hepatic cirrhosis
-Endocrine : acromegaly, gonadal hypofunction;
-Recurrent parotitis of childhood;
• Unilateral Salivary gland neoplasm;
-Bacterial infection;
-Chronic sialadenitis.

2. DRY EYE
-Inflammation : Stevens-Johnson syndrome, pemphigoid, chronic conjunctivitis or
blepharitis, Sjögren’s syndrome;
-Neurologic conditions : impaired lacrimal or eyelid function;
-Toxicity : burns or drugs;
-Miscellaneous : trauma, hypovitaminosis A, blink abnormality, lid scarring,
anesthetic cornea, epithelial irregularity.

3. XEROSTOMIA
-Viral infection;
-Irradiation;
-Diabetes mellitus;
-Psychogenic;
-Drugs : antihypertensives, parasympatholytic, psychotherapeutic Trauma;
-Sjögren’s syndrome.
